Patient Responsibilities
As a patient who received medical equipment in your home,
you have the responsibility to:
- Provide a complete and accurate medical history. This history should include all prescribed and over-the-counter medications that you are taking.
- Inform us about all treatments and interventions that you are involved in.
- Follow the suggestions and advice prescribed in a course of treatment by your Health Care Team.
- Understand that, if your refusal of treatment prevents us from providing care according to ethical and professional standards, we may need to end our relationship with you after giving you reasonable notice.
- Be considerate of the rights of other patients and your Heath Care Team personnel and property.
- Provide information about unexpected difficulties you have involving your health care.
- Make it known whether you clearly understand your plan of care and the things you are asked to do.
- Make appointments and arrive on time. You must call us in advance when you cannot keep a scheduled appointment.
- Provide us with correct information about your sources of payments and ability to pay your bill.
- Ask questions about any part of your care you do not understand.
- Inform your Heath Care Team if you are experiencing any problems with your equipment.
- Follow your Heath Care Team's instructions about the appropriate use of the equipment.
- Follow equipment instructions regarding the handling, are, safety and infection control procedures (general cleaning) of the equipment.
- Use reasonable care in the use of the equipment provided.
- Immediately contact your Health Care Team on equipment which is life threatening, when the equipment is dewfective or not functioning properly.
